A few spam messages aren't hitting the "hit every IPv4 address" RBL used
for testing.

Okay, mine didn't hit because they were internal hotmail spam sent by
hotmail staff to my account, so there was no Received: header.

Theo's was also lacking a Received: header.

lan - can you look at yours and see whether they had Received: headers?
(You can just attach them to this bug if you want.)
