Hi

I'm using the attached foo.xml docbook to generate foo.fo with xsldocbook. Then
I transform that with FOP to foo.pdf which seems to have the following problems:

+ The Table of Content contains 14 entries but only the first two are clickable.

+ there's quite a bit of space between section header and list of items beneath

+ the index at the bottom looks a little bit strange - the first names
(secondary) are not indented as I'd expect)

sorry, disregard, I have to test this with the latest changes in the code
repository which I thought I had but in fact didn't

Set to invalid for now

batch transition to closed remaining pre-FOP1.0 resolved bugs
