Java: 1.7.0; Java HotSpot(TM) Client VM 21.0-b17
System: Windows 7 version 6.1 running on x86; Cp1252; en_US (nb)

Steps to reproduce:
Start NB with fresh userdir -> go to services tab -> invoke popup on Servers node -> select Add Server -> in the Add server instance wizard select some server other than glassfish (jboss, oracle weblogic...) -> click next -> wait for j2ee to activate -> click back -> see the result - see attached screenshot

This is ergonomics issue.

Is not it more likely the dialog is not ready for changing the set of providers while being displayed?

As I said, imho the primary problem is that the dialog is not ready for list of providers to change while it is open.
