related to custom action definitions

could any from the reporters please attach the nbactions.xml file from a project on which this problem occurs

A file with just this seems to always trigger the bug:

reproduced.

the question is how it happened that nbactions.xml is in such state, did you edit it manually or do you know how to reproduce it?
