Build :06082006

Add Dataprovider button is missing in Table Layout dialog.

Evaluation:
Unfortunately we have to remove the button from Table Layout and other biding
dialogs, becasue it depends on Server Navigator for adding the dataprovider to
the form. Since we no longer have the Server Navigator UI, we have to remove
this button.
