I used the template for an OOo3-Presentation. There are two master inside the
file. I created four slides. The first and the third out of the first master and
the second and the fourth out of the second master. If I change to the notes
view, there are no slide visible on the notes page of slide four.

My environment: openSuSE 11.1-x64, OOo 3.1-i586, KDE 4.2.2.

I can confirm this problem for the attached document on WinXP.

Setting target to OOo 3.3.

The notes preview on the fourth slide is not missing, it is just very, very
small.  The second notes master page description in styles.xml specifies the
size of the preview object to be 0.001cm by 0.001cm.

Whether this value has been set on purpose, by accident, or by program error, I
can not tell from the available information.
