Stating "failed" for a test in TCM means giving an issue number - so far, so good. But how the old and 
solved issues get removed from the test description? One could do that 
1) automatically by removing all "solved", "invalid" or "closed" issues from tcm time to time.
2.) manually by adding a "remove from test case" button. This could be done in "Test Scenario 
maintenance" > choose a test scenario name  > click on test Id, which is a link  > in the left upper 
corner of the test description there is a link "Bugs for this test case" - in the list which is opened by this 
link the button could be placed.

btw: This list of bugs as well as the removal button I normally would expect to find  in test case 
maintenance.
