After creating tables in a new database the "create form in design view" and
"use wizard to create form" tasks are disabled.  Also, when attempting to 
create a form at the end of the create table wizard OpenOffice crashes.

Could you upload example file?
Please give more details, what type of odb file you used?
What table structure caused this problem?
