PDF export and font subsetting don't work with codes outside of the unicode base
plane. As far as I see it pdfwriter_impl.cxx, salgdi3.cxx and sft.c need worked
to work with surrogates

I'm sorry that there are not enough resources to change this behaviour for 2.0.1

Status update: The original problem here got fixed long ago (issue 45983). It was broken again with CWS 
glyphadv though.
