I would like to have the ability to leave out the header/footer on certain pages
(such as the title page) while still having it in the others; or have the
header/footer *only* on specified pages; or to be able to alter header/footer
for a single page while other pages remain the same

according to

reassign this issue to requirements and set the status to new

Looks like this enhancement is addressed now (OOo 3.1 and 2.4) using page styles
or perhaps I didn't understand the request?
