- open the attached database
- open the report "countries, tabular" for editing
- note there's a formatted field in the upper left corner
- delete this field
- save and close the report design
- re-open the report for editing
=> the control re-appeared

This is a new implementation in CWS oj14. Adding keyword "new_implementation",
targeting to OOo 2.x.

Note: the problem vanishes when you remove the shape from the page header. Seems
that importing the shape somehow creates the ghost field.
