Case cenario:

Picture/table caption - Text has reference to endnote.
Eg:

Figure 1 - Something (adapted from [1])


Index will not show the "1" endnote reference, but instead:

Figure 1 - Something (adapted from [])

This issue only exists with the following conditions:

1) Number is a cross-reference

2) Cross-reference has format "Reference".

Same problem with footnotes.

The footnote anchor is not displayed in the TOC.

Reset assigne to the default "issues@openoffice.apache.org".
