After problem described in Issue #94371, when OO is started, Document Recovery panel appears asking 
to recover document open in previous crash. 

When it is recovered it says "Click "Next" to get to the Error Report Tool"

When I click "next" I get to the recovered document, and can't find the "Error Report Tool"
