I have a document which contains the same spelling error a lot of times.  While
checking spelling, I hit ‘change all’, however, later I see the same error pop
up again and I have to confirm again.  It seems as if the program ‘changes all’
only within a secret selection which it is controlling, but which is not
indicated.  If I click next to the spell check dialog, I see this selection
flicker and disappear.

More detailed instructions: open the attached document (language Dutch has to be
installed), do spell check.
Click ‘change all’ if it suggests to change ‘anthroposofie’ to ‘antroposofie’. 
Then you decide to do something else in the document first, like correct an
error you saw which the spell checker can’t find, so while it shows you that
‘wooren’ is incorrect, you hit ‘Close’.

The ‘anthroposofie’ on the last line will not be corrected.

Another use case: put the cursor on the heading, check spelling.  It will find
‘anthroposofie’, hit change all, but say no the the question to continue in the
beginning of the document.  The first two occurrences remain.

I believe this problem is the same as reported here:
  Issue 103158: The spell checker ' Change All ' option does not change all
occurences
  Issue 110688: spellcheck: Change All button does not work correct

Issue 103158 is marked as a duplicate of Issue 85936, but that seems like a
different problem as I read it.

The problem is that clicking "Change All" does not actually make all the changes
when you click the button. The label suggests that clicking the button will
apply the correction to the entire document, but if you close the spellcheck
before it has searched the entire document, then some corrections may be missed.

There are now at least two open issues for this problem: this one, and Issue
110688, and another that is incorrectly marked as a duplicate.

I also have this problem. I have multiple instances of the same spelling error, and when I use "change all", only the first instance changes. The same spelling error comes up again and again as I work through the document.

This occurs using both English UK, and English USA.
