to reproduce:
Insert a CheckBox in a form in base. The default style property is "Flat", but the control has the 3D style.
Change the style to 3D. The control has the flat style. You can do it only once.
The same happens with RadioButtons.

This has worked at least in OpenOffice 3.2, so it's a regression.

BTW, in writer and calc the 3D style isn't available anymore for checkboxes and radionbuttons.

Effect might be related to 2 LibO Bugs, we should check!

@eric, @ eberlein:
Can you compare observations here with the observations in Issue 124499? I am pretty sure that it's a more general problem than "Base only" or "Writer only"

A Sample document (Base: connected to Biblio database) would be fine.

I'm running Windows Server 2008.

I confirmed that in writer, the problem I reported also occurs with checkboxes, like this bug reports.

I checked Impress and Draw and the same issue is present there.

I have no experience in Base, and I couldn't find a way to enable the Form Controls toolbar, so I'm unsure how to confirm the observations as it relates to Base.

I did some test with  Attachment 46029 [details] Details for Issue 78522 and with server installation of "AOO 4.1.0-Beta – English UI / German locale - [AOO410m14(Build:9760)  -  Rev. 1577236 2014-03-15]" on German WIN7 Home Premium (64bit)", own separate user profile. My Win View setting is "WIN7". I was not able to reproduce problems with that document:

1. Open sample document
2. Open 'Forms -> Rightclick "DOKUMENTE" -> Edit'
   > FORMS view opens
3. If necessary activate Toolbar "Form Controls", activate Edit mode and
   try, for example one by one alternating 3D - Flat - ... With my settings 
   View followed exactly 3D- Flat setting
4. Close - reopen in View mode: 3D and Flat still ok.

But I am rather sure that OS and OS  settings have influence.

Can you contribute a simple sample document what also contains some screenshots with comments and a view on your Desktop?

Following my own instructions in comment 1 I could reproduce the same behavior with Windows 7 and AOO 4.1.0:

New odb without any data, new form, new CheckBox: PropertyWindow shows "Flat" (default), preview shows "3D" and vice versa. This stays when closing "Edit Mode".

And, once again: Writer, Calc and Impress don't have 3D Checkboxes, so Product=General is wrong. Maybe because of issue 105717 (CWS controltextrendering), which was introduced with oo3.3.
