Printing envelopes causes incorrect document/paper size error on printer

Since upgrading to v4.1.0 when try to print envelopes the printer (HP OfficeJet 6500) gives an incorrect document/paper size error and does not print. Envelope is passed through the printer without printing. #10 and #6-3/4 were tried with every combination of orientation on the Envelope dialog. Envelopes were tried in printer in both landscape and portrait orientations.
Note: I created a #6-3/4 envelope in Wordpad and it printed with no problems.
I've tried to print #10 envelopes. When I go to File-Print, it shows it is going to print a C5 envelope, not what I wanted. Having spent 40 years in main-frame programming, I would venture to guess that there is something screwed up either with the envelope characteristics table (or whatever you call it) or the addressing/indexing/lookup into the table. Unless of course, it is the contents of the "#10" entry which are screwed up. This is especially annoying since the #10 envelope is probably the most commonly used. While I am retired, and not a commercial user, I DO have to send business-type letters from time to time, and hand-written envelopes just don't hack it... I am amazed that this problem, which should be rather easy to fix, has lasted so long and has just a "normal" priority. Another little fix, while you are at it, is to allow me to print an envelope WITHOUT having another page involved. A "Print" button on the envelope window should be rather easy, too, as well as the provision of a warning when the printer needs the envelope. The above are "nice to have" - they would make printing envelopes much easier. The fix for the envelope size is vital: this bug makes using OO to print envelopes very difficult, indeed.
