Square root is not displayed correctly. Instead of the sign the part of the text
in brackets becomes underlined.

Simply type sqrt{x+y} to test it. Export to pdf doesn't change things.

This is a terrible bug. As for me, this makes using of the OO version 1.1 beta 1
impossible at all, because I use formulae in my documents. So the bug is critical.

I can't reproduce this.
It may be a font problem.
Please attach a screenshot of that what you see.

Judging from the screenshots only it is different.
This one seems to feature also (only?) an error in positioning the 
symbol too much down and right...

TL->MRU: To you in order to try to reproduce.
Without that I cannot fix it.

Describes the same as #12557. Opensymbol font in OO 1.1 Beta is messed up.
Workaround: use OpenSymbol from OO 1.0.x.

Dear MRU,

I appreciate your effort.

It appears to me that the suggested workaround does not apply to issue
12557 on Linux, of which this issue on Windows is marked as a
duplicate. Could you look at recent comments to issue 12557?
