Steps:

1) Create a new text document.
2) Add a few paragraphs.
3) Insert a graphic from a file: Insert | Graphics | From File
4) Add a paragraph after the graphic.
5) Click on the graphic.
6) Choose Insert | Graphics | From File from the menu.

Observed: The Graphics dialog opens.
Expected: To be prompted to choose a new file, which would replace the selected 
graphic.

This is not a defect, Type is changed to Enhancement.

Why is this not considered a defect???

If I select the menu option "Insert image" I want to be able to do what that
menu promises: insert an image, not editing the existing image properties (if I
wanted to do that I would have chosen Format - Image).

So this is clearly wrong. 

This bug still exists in version 2.3.

I just reported Issue 101706. I think that the behavior in OOo is inconsistent
when you have a look at the different applications.

I agree to simonbr. This issue is a DEFECT not an ENHANCEMENT.

The behavior is wrong.

If image is linked, current behaviour allows to replace it by another one.
So, for me it's not a defect but an enhancement.

This issue has a lot of duplicates and I agree that the inconsistency with other
applications as well as the "surprising" implementation deserve an
investigation. I agree that this has to be treated as a defect. That does not
mean that it isn't one, but it's either correct behavior (and so it should be
"WONTFIX") or it's rubbish (and then it's a defect).

So I've set the target to "3.x" and grabbed the issue. I would be pleased to get
an explanation why Writer does behave that way.

IMHO this is in any case a bug. 
Note: The behaviour with drag & drop is another.
Both behaviour should be equal.

I got this on MacIntel and filed issue 106340 (which is a dupe  -shame on me) but some comments 
from that:
If the picture is linked, one could change the link in properties dialog and so replace it. Nevertheless 
program behavior should be consistent and changing just the link should be regarded as a second way 
to do the same thing.
In Draw it's working fine as expected - means: File open Dialog opens. But then the selected image gets 
not replaced but selection is ignored and the newly inserted image gets just placed over the old one 
and does not replace it.
Under XP Insert > Picture > From file… is inactive when a picture is selected which also is not the 
expected behavior.

Btw. It wouldn't be a defect, if the menu Insert - Picture was shadowed and
unchoosable while there is a picture selected.

But it would even better to change the behavior of writer in the mentioned way -
to get the possibility to replace pictures.

I have the same request.

Recently I switched back from using OpenOffice 3.3.0 back to using version 3.1.0 just because I want to be able to replace graphs in my presentations, and the most recent version just inserts a new image instead of replacing the selected one.

When I need to make a presentation on a progress in my project, it is very good to start working on the presentation in advance. I put into the presentation the preliminary results and the day before the presentation I want to be able to update the plots with the most recent results. In version 3.3.0 I had to remake essentially the whole presentation from scratch, since all the formatting has to be redone, while in version 3.1.0 the task is easy, since the selected plots are just replaced in place.

I hope you will get the feature back in the new releases.

Notice, that the problem is not only in Writer. But the behavior of Insert in the case a picture/image/graphic/bitmap (or how you call it) is selected, is inconsistent between the modules and not as users expect.

The list of duplicates is actually longer, because some of them have references to other duplicates. The length of this list shows, that it is a severe usability problem that should be solved soon.

Mathias: The issue is still assigned to you. Are you willing to work on it? Otherwise please set it back to default.
