In word chapter numbers appear in the index followed by a '-' seperator followed
by the page number. If there is no chapter number the seperator '-' is not
shown. In OpenOffice there is no way to replicate this in the filter. If we
create a '-' text token it will appear regardless of whether or not there is a
chapter index. We need some kind of conditional seperator in order to support
chapter numbers in indexes.

mmaher->ama: The bigger problem here is that we do not support chapter numbers
in page numbers. In word you can go to Insert->Page Numbers->Format and click
"include chapter numbers". From that point on the Page number field will always
have the chapter number before it followed by a seperator followed by the page
number. Rather than trying to get the filter to simulate this in the index it
might be better to support chapter numbers in page number fields. What do you think?
