If you export an OO-Form (680m49 with pdf01) to PDF the hierarchy information of
your form-fields are lost.
->OO-Document with hierarchy (forms) -> PDF export -> plain PDF

The current code only supports a flat widget structure in the resulting PDF;
also form layers are sort of virtual as fs told me; we'll have to redesign for
form structure to be exported to PDF.

it would also be nice if you could specify the hierarchy using dotted notation
as with Adobe Acrobat, e.g. two fields "a.a" and "a.b" define a node "a" with
two children "a" and "b".
