When inserting a page number field from the fields menu into the header/footer
of a document, you can number the first page of a document from 1, next page is
2 and so on. 

If I want the first page to start at 21, for example, my understanding is that
the field offset must be +20. When I entered +21 for my offset value, the first
the page number in the field disappeared completely. This problem also appeared
in the latest 2.0m series build as well.

Am I understanding the use of the offset to the page number field correctly? I
am not using master documents.

No, your understanding is wrong.
The offset '+21' means: Do not show the actual pagenumber but the pagenumber of
the page, which is 21 pages further. If such a page doesn't exist nothing can be
shown.
To start the counting of the actual page at 21 do this: Go to the first
paragraph of the page, go to contextmenu, choose paragraph. In tab "Text flow"
enable "Breaks" and "With Page Style", choose the page style of that page and
set "Page number" to 21.
