Windows XP SP2.

The document has a header and footer on each page.  Insert the chapter name 
field in the header.

The first page is a title page with no chapter name.  The second page contains 
the first chapter.  When creating the document it correctly leaves the header 
page chapter name field blank.

When the document is loaded the chapter name field on the first page holds the 
chapter name of the last chapter in the document rather than being blank.

Exactly the same happens in version 1.1.4.
