On OOo 2.2, creating a table of contents with hyperlinks is not working.

I followed the tutorial here:
http://www.tutorialsforopenoffice.org/tutorial/Create_and_Modify_A_Table_of_Contents.html#3.10.Add%20Hyperlinks%20To%20The%20Table%20Of%20Contents|outline
but the entries are not linked.

I created a simple document which should have hyperlinked toc entries --
following the tutorial -- but the entries are not linked. I will attach the
document.

The TOC is not based on outline styles but on index entries. OOo is yet not able
to set Hyperlinks to index entries. The concerning feature request is in issue 3356.
