Extension menu not initialized after install

To reproduce: 1. Open blank Writer doc in OOo 2.4.0 2. Load Wiki Publisher 0.9.2 into OOo 2.4.0 3. Examine File->Send menu Results: Wiki publisher missing Expected results: Wiki publisher visible Note: If you create a new document, it's visible in the second but not the first. If you close and reopen document first document, then you see it. Seems like a refresh is necessary or an event was not processed?
This is a common problem to all extensions. One should even advice to restart OOo completly, just like Firefox does.
Life deployment not always works, therefore we should provide a new entry in the description.xml, indicating that a restart is required.
Changed title, issue type.
Using the enable button is also a form of life deployment. In this case the user should also be displayed a message, that one needs to restart OOo.

Back to the original problem: If you create a new document, extension supplied menu items and toolbars are visible, but not in documents that were already open before installing the extension. If you close and reopen the "old" document, then you see it. Proposed solution was to do an office restart to work around the actual bug. Why this? We should do as always and fix the actual problem. If we see demand for restart-after-extension-installation feature, we should create a separate issue for this, not highjacking a bug report for a related, but actually different problem. cd: as this actually is a framework issue (i think), please take care.
set target to 3.x since not release relevant for 3.4 release.
