Hello;

The attached file was created with POI. As best as I can tell, it follows the 
sample exactly. However, if Excel has all of it's standard Add-Ins enabled, 
loading this file will crash Excel 2000 100% of the time and Excel 2003 95% of 
the time.

Please try with the latest and greatest POI-3.7 or POI-3.8-beta3. If the problem is still there, please post an isolated unit test that we can use to research the problem.

No update for a long time => resolving this bug for now, please reopen with some unit test code that we can use to reproduce the invalid file being created by POI.
