Using graphics in a word document (successfully inserted by 'strg+c and strg+v) there sometimes (but contantly) after a while appears the announcement 'read fault error'; this happens, when working in the file (writing, formatting ...); 
then, when I save the file without noticing the error, the graphic ist gone;
I'm working since years with Open Office, and this kind of error first happened, since using version 4.10 - before it there never was a problem with this ...

Could you please check our recently available developer snapshots for our planned 4.1.1 release - https://cwiki.apache.org/confluence/display/OOOUSERS/Development+Snapshot+Builds

We had made certain bug fixes in the area you mentioned - see issue 114361, issue 124717, issue 124946
