I submitted this form by fax to ASF on 29th October 2003 at 10:02 UTC.
Can you ask them to double-check whether or not they did receive it,
please.

Hi Martin -- yes, we did double-check -- they didn't get it :(

We're checking with them now, as there's quite a few names that aren't on the
list that should be.  It could be we'll require another copy -- but
sign/scan/email will probably be easiest and I think they'll accept that.
