Build 0421
Windows XP

1. Set Tools > Options > Miscellaneous > JavaScriptDebugger Suspend on First Line
2. Run Debug File
3. Expect to be able to step through with Debug Toolbar enabled etc. but debugger does not stop

Tried with that single option activated alone and with others.

Hmm...seems to be an issue with firebug. It is not firing the onTopLevel callback of the register listener. We may have
to punt this to M2 and solve in a different way.

Use the showContext() callback hook to implement Suspend on first line option.
