Using version 2.4 of the org-netbeans-modules-a11y.nbm.
I'm running into this problem of "Error : test cancel" when I run the test.

1) Create a New Project select Service Oriented Architecture/BPEL Module
2) Select Finish
3) Select the Process Files folder.
4) Right mouse button click to New/BPEL Process...
5) Select Finish
6) Select the BPEL Mapper from the Window tab.
7) Select the empty box in between the Process Start and Process End.
8) Select the BPEL Mapper window.
9) Assuming you have the A11Y tool already up and running.  Make sure in the 
Test results tab, you have the File option selected to some file on the hard 
disk.
10) Press the Cntrl-F11 key.
You should get the Error : test cancel
