Hi.
If you use a page number offsite + a index/table of content, the page number in 
the index is always the original one.

To reproduce it:
1) Create headers in some pages,
2) Create a index in the first page (Insert-> Indexes and Tables-> Indexes and 
Tables->)
3) Add a footer page counter (in the footer: Insert->Field->Page Counter)
4) Go to the footer and select an offset to the page counter (RMB + Fields-
>Offset)
5) Update the index/table (RMB + Update Index/Table)
6) it doesn't update the page counter in the table:/

Same root cause as issue 12138.
Use page number manipulation via "Format.Paragraph.Text flow".
