the formula commands

'hat a', 'bar a' etc.

produce boxes instead of the desired hat, bar, etc. This does not depend on the
font used.

dots, ddots seem to work instead.

This is most likely a duplicate of issue 113586.
tl->antares: please look for the listed work-arounds in the above mentioned
issue (see comment from 'Nov 17 08:02:58').

Does it help when you copy your OpenSymol font the OOo installation to the
systems font folder?
