I experience this issue in all openoffice versions including 3.4

I tried to replicate this bug on Mac OS X (version 10.7.4).
When opening your document (Bonding1.doc) with OpenOffice 3, the header and body content were indeed mixed. However, I copied your document in a .pages file on Mac, fixing the header, footer, and just copying the rest of the content.
 I then exported it to .doc. When opening the new .doc file with OpenOffice, the format was actually correct and no content was mixed. I included snapshots of what the document looks like on my computer. 

Was the Bonding1.doc created with OpenOffice or another application? It seems to work fine when opening a .doc which was created through Mac Pages.

Are you still facing the problem with mixed header, footer and bodycontent? I believe that you first of all should contact our Community Support Forum for this since this might be a question for them.

If you're not pleased with this answer I must say to you that there is not enough information here for me to test your scenario. Specifically I need:

You need to describe your steps when making your sheet. This in order for me to understand your defect. 

As I understand you work with Windows?

I put needmoreinfo as keyword.

When I opened your document, in Word. Some of header footer and body contents are mixed. But only in page (after 3)
 
I corrected the text an saved it. 

Opened in Win7 Word 2007 - all saved and applied properly  as I saved previously.

But when I opened it thought AOO 4.0 - When opening your document (Bonding1.doc) with OpenOffice 3, the header and body content were   mixed.

But when I create new document in in Win7 Word 2007 , fill headers and footers and body pictures, and in headers and footers too. Saved it and open in AOO 4.0 all headers and footers and body text, pictures are saved properly.
