Document that causes the crash

OO Writer is constantly crashing when scrolling in the attached document. Most often to or from the last page.

Usually Apple shows a dialog after a crash, asking if you want to report it to
Apple; if you select to view the details of the report, it will display very
useful information; if you get the Apple report dialog, please copy that crash
report detail in a text file, and attach it here (but don't send the report to
Apple).

I am not able to reproduce the described defect under Windows 7 (64bit)
--> defect seems to be depending on the system

I tested the document with version 4.1.1 on windows 8.1.
The program did not crash under these conditions but it was noticeably slow and did not scroll smoothly at all. There also seems to be an error in the field placement on page 14 for me. I am not sure if the placement this is an issue in the document or open office. 
To further test I copied and pasted the documents contents twice in an attempt to make a larger and more form heavy document than the original. This file scrolled even more choppily but still did not crash. As for the form placement noted on page 14 of the original file, those forms retained their odd placement in both occurrences on the second document.

Tried to reproduce the defect in OpenOffice 4.1.0 on Mac OS X Version 10.9.4
Was not able to reproduce the bug even after pasting the same content 10 time.
Was able to scroll through the pages swiftly using the touch pad without any crash.
Think that the reported bug is a very rare one. 

On further tests run on the document, it was found that when the user scroll the document using the scroller, the scrolling was not smooth. At times, moving the scroller does not resulted in the actual scrolling of the document.

Steps:
1.Open the attached document.
2.Copy and paste the content 2-3 times.
3.Try to scroll the page by placing the mouse over the scroller and moving it.
4.Observe the result.

I could not recreate the issue.  When I was trying to recreate, I did have severe lag moving the pages up and down.  It got to the point where it just stopped.  BUt when I would restart scrolling it would scroll again.
I was using my mouse wheel to scroll and also the menu scroll bar.
1. Open Document
2. Scroll up/down with mouse wheel.  Lag but no crash.
3. Scroll up/down with menu scroll bar.  Lag but no crash.
4  Scroll with Page Up/Down.  Lag no crash

I tried to test this bug on mac OS and windows operating system. I was unable to replicate this bug. 
Test 1:
Environment:
OpenOffice version 4.1.2
Windows 10 64-bit
build: AOO413m1(Build:9783) -  Rev. 1761381
Attempting following:
1. Download the file given by the original reporter. (Prozess-Audit.odt)
2. Open OpenOffice.
3. Open the downloaded file.
4. Scroll the page by scroll bar from top to bottom.
5. Scroll the page by scroll bar from bottom to top.
6. Scroll the page by mouse wheel from top to bottom.
7. Scroll the page by mouse wheel from bottom to top.
Result: There is not a crash on the window version for this test.
Test 2:
Environment:
OpenOffice version 4.1.2
macOS 10.12.3
build: AOO413m1(Build:9783)  -  Rev. 1761381
Attempting following:
1. Download the file given by the original reporter. (Prozess-Audit.odt)
2. Open OpenOffice.
3. Open the downloaded file.
4. Scroll the page by scroll bar from top to bottom.
5. Scroll the page by scroll bar from bottom to top.
6. Scroll the page by using two fingers sliding on touch pad from top to bottom.
7. Scroll the page by two fingers sliding on touch pad from bottom to top.
Result: There is not a crash on the mac OS for this test.
This bug is important to customers. Because the scrolling function is one of the most important function for users. When user either editing or reading a Writer document, scrolling function must be used. If the bug is existing, it is a huge functional problem. And also, table use is a important function for a Writer document user. If the bug for heavy table use occurring, the software will lose huge number of customers.

I was unable to reproduce this crash on OpenOffice 4.1.3 Writer (Build 9783) using both Mac OS X Version 10.12.2 and Windows 10 Home Version 1607. I did experience some lag on both systems, but the application never crashed. However, I did notice that this lag increased greatly when my cursor was inside one of the form fields. On both systems, it would be unresponsive to scrolling for about 5 seconds before scrolling correctly. If the cursor is outside one of the form fields, I experience much less lag on both machines.

Many users don't able to reproduce.
