If the users chose an offset for page numbers (i.e. if a document contains a
cover page which should not be counted as page), the function for
auto-generating of indexes fails and always creates indexes without offset.

Offset doesn´t change internal page number. But only internal page
numbers are read by various references and tocs. See Issue 12138 for
details on how to change page numbering right.
