When importing a Word 2000 document, an extra chapter appears that does not
exist in the original document. See attached file for an example.

I can Reproduce the problem on 
OpenOffice 1.1 RC4(default Install, US), Win XP Pro Sp1. 
(And MS Office XP Sp2). 
It is real problem

When opening the doc file in MS Word and OOO writer, look at page iii, 
below 2	Second chapter ....
there is additional (which is not exist in original doc)

MRU->AMA: for OO 2.0 CMC already implemented, that these kind of
Heading paragraphs do not have an Outline number. Now we also need a
possibility, that the TOC creation takes care of this...

At the moment we list every not empty paragraph (with heading style)
in our table of content. An empty paragraph is listed even it has a
number. We need a compatibility flag. If it is set every paragraph has
to be in the table of content which has a numbering or contains not
only whitespace characters.
But I don't think that empty or nearly empty paragraphs are often used
with heading styles, so we should fix this for Q with lower priority.

While hardly an expert on OOo, I do not believe this to be a TOC
issue. The extra chapter should not be created in the document in the
first place, but once it is created the TOC picks it up which I guess
is correct behaviour.

As for where the extra chapter comes from, possibly it exists in Word
as an empty (or blank) piece of text that was accidentally marked up
as having paragraph style "chapter". Word ignores this empty markup
block, while OOo treats it as a regular chapter.

MRU->HBRINKM: have a look at the paragraphs at the end of the docuement. It is
formatted with "Heading 2" and only contains a blank. It is listed in the TOC
and also with a number!
