Customized CVS comment form is not migrated

If you commit a change to CVS your editor is started to enter a comment. The form from the 1.1.3 production site is not migrated to the staging machine. I will attach the 1.1.3 form, the new form and a "merged" one.

The template contains DOS line ends (CR and LF). Please use Unix line ends: LF only.

Fixed. File updated in the instance set. The corrected form will be applied during the production upgrade.
We would like to verify issues. Please let us know when an OOo server is available to verification before the production upgrade.
file format is nearly correct now, but one CR/LF has been overseen: the enter cvs log section seems to be autogenerated.
