The dialog "Insert - Note" shows an "Author" field which reflexes who wrote the
note.
Unfortunatly, by now, this Author field is overwritten by the name of those who
edit the note afterward (see issue 32157).

Other problems arise in this dialog:

1) The dialog should have a second field "Last edited by:" which would show the
initials of the last editor of the note.
2) Change the button Caption from "Author" to "Edition info"
3) remove the "Insert" fix text which is useless and move the [Author] button
the the left, aligned with the Edit field, for better design.

In the new implementation: different UI, quite the same solution, but...

Please have a look at the mockup I will attach.

When an other author adds a comment on a comment, name and date are
automatically inserted and fixed as a header.
