Two manifestations of this bug:
1) A MS Word doc imported with Heading# styles, which was converted to numbered
items.  See

and
http://www.math.umd.edu/~dcarrera/preprop_en.sxw,
Daniel Carrera's conversion of this with OOo1.1.3, which imports in 1.9.58
incorrectly.
2) Heading styles in HTML document are output with numbering turned on.  See
http://family.needleswebs.com/index.html, for an example (text in first cell)

MRU->DVO: saving the mentioned doc to sxw or oot format will erroneously assign
"Heading 3" to 3rd level of outline numbering in this case. See the table cell
where all the checkboxes reside.
Use the shorter document I'm going to attach.
