1.Open Openoffice 1.9.62 calc
2.Click on Data Source Explore window
3.Right Click  on biblography 
4.On the left side click Forms Icon
5. Click "Use Wizard to create form"
6. Window for Form Wizard Opens (with 8 steps)
7. step 1 - select the field of the form & along with the table name
8. step 5 - Arrange Controls (format of displaying form)
9. step 6 - set data entry (have 2 options)
10. Select the first option - The form is to be used for enter new data only 
exit data will not be displayed
11. step 7 - apply styles
12. step 8 - set name (give the name of the form of your option)
13.Openoffice 1.9.62 calc with form design is displayed
14. enter the data coloumn wise
15. at data navigator click next record
16. We cane enter the record .. The tab setting is not at the first coloumn but 
it is at the last coloumn
  Ex : here I created form of Emp Table with three coloumns 
       Ist coloumn empid
       IInd Coloumn empname,
       IIIrd Coloumn DU(delivery Unit)

clu->virtdb3: please try it out in a current version (wizard is reworked) and
check if still occure: http://download.openoffice.org/680/index.html
