When importing the index pages comes out with only one line and the rest is
blank. File attached.

This is already fixed in newer builds of OO 2.0, like 680m93. OO had a general
problem that the outline information was lost after opening a file.

I had a closer look at the document. The Index will be empty in OO Writer,
because OO could not find any index entries in the document (text has ben deleted).
If you update the index in Word it will also be empty

THe index here is empty, because Word's RD field is used here. This tells the
index that it should be build up on information from external files. This option
is currently unsupported in OO Writer, thus the index will be empty when the
attached document is opened.

one of the latest versions of OOo, as the problem(s) may have already been
