I use OOo1.9.113 German language pack.

Open help modul Writer, tab "Inhalte", chapter "Textdokument", part "Befehle und
Menüs", part "Menüs". You will see an item "Hinzufügen". This belongs to the
English item "Insert" and should be "Einfügen" in German. In the UI and in the
text, which belongs to this heading, it is also the term "Einfügen".

I cannot locate this string. 
Do you have any more info? ie. keyID, par_id, hd_id, module name, etc?

It is an error in the file "swriter.tree" in the directory "help\de"
In that file you will find it inside the node <node id="020201" title="Menüs">
in line
