Base, Form Wizard
Same as closed issue 47483

Table AddressTable defined as:

KnownFrom	Text[VARCHAR]
SendCard	Yes/No [Boolean]

When Form Wizard is run, SendCard field is not shown on Field Selection field.

Resolve issue, mark it as duplicate of issue 47418. 
The mention issue is about the report wizard, but it's the same error.
