Reproduction:

1. Open Tools/Customize
2. Switch to tab Toolbars, select standardbar
3. Select any item
4. Click Modify/Change icon...
5. On the "Change Icon" dialog click Import...
6. Select an icon (e.g from here:
http://www.openoffice.org/nonav/issues/showattachment.cgi/42042/ooicons.tar)
7. Import it
8. Repeat steps 5 to 7 with the same icon
-> The icon appears in the palette twice.
