This bug was reported to me by a user and confirmed in ooo.org 2.3.1 and 2.4.1
in both Writer and Impress. 

User attempts to customize the toolbar by the standard method of
Tools->Customize->Toolbars. The user is attempting to insert "Text Direction
from Right to Left" and "Text Direction from Top to Bottom," from the "Text
Object" toolbar. However, the icons will not show on the toolbar. The toolbar
will flicker and show the icons very briefly, but will instantly disappear.

You need to activate the corresponding language option in Tools|Options →
Lanaguage Settings (see issue 54423 for details
