Sometimes the footer is not seen on certain pages.  I suspect it has something
to do with an autogenerated TOC, but I may be wrong.

I forgot to mention.  This DOCX file displays perfectly under MS Word 2007.

That the footers are not available cannot be reproduced here. But: the page
numbers in the footers are incorrect.

MRU->HBRINKM: see attached document. The page numbers should be restarted at the
beginning of the TOC section and right after that section. Also the page numbers
in the TOC section should have "i, ii, iii..." format.
This works fine when imported from .doc format, only docx import is affected.
