OSL_ENSURE in FmFormPageImpl constructor (svx/source/form/fmpgeimp.cxx:96) is
triggered when the previews in the master page panel of the Impress task pane
are created.

To reproduce 
a) start the Office,
b) create Impress document,
c) click on "Master Pages" in the task pane.
