I just installed v4.0.1, created a odt file and tried to export as pdf. Writer crashed with this error Problem signature:

I opened file. Then Export as PDF. I tried both as a PDF/A and tagged. same result both times. This worked fine in previous version of Writer.

Reproduced with given sample document with AOO 4.0.1 under Windows 7.

Not reproducible with AOO 4.1.0 Beta.

Searching for the issue which fixed this crash...

Seems to be fixed by the changes made for issue 123409

Open the sample document in AOO 4.0.1, delete the image, export to PDF with PDF/A or tagged.
--> no crash.

Works well in current trunk version, but asserts

Error: document structure in disorder ! From File c:/aoo/svn_trunk48/main/vcl/source/gdi/pdfwriter_impl.cxx at Line 11196

Taking a look what this may mean...

Yes, I removed the image and was able to export successfully. I will try a different file type for the image. It is a jpg and will try with a png.

I will also try another document that has never failed with a jpg masthead.

I made sure that the image was at the front and had no wrap applied. The export worked correctly.
