Had this issue since I started using Open Office many years ago while making newsletters:
when using images as header and/or footer in a document, they suddenly disappear, never to re-appear again. I notice this happens in particular when having been away for a while from the computer. At the location of the images there is only indicated  "leesfout" which is Dutch for read-error; the place-marks for the image are still in place. I suspect it has something to do when the file is automatically saved.

This bug is independent of the Windows version (it has occurred in XP, Vista and 7), various computers and laptops and also independent from the Open Office version. In my view, a very persistent bug.

Only opening the document in Word shows the images again... I usually do this to copy and paste the images back into the OO document just before I convert it to pdf to make sure it does not keep disappearing.

You issue must be reproducible by the developer team.

A sample document or further information on the inserted images (linked or embedded; file format of the image - PNG, JPEG, ...; ...) would help.

We had solved certain issue in this area for the coming 4.1.1 release - see issue 114371 or issue 124966 or issue 125289

I could not replicate this on OO v 4.0.1, Windows 7 O/S. The original report did not list specific steps to replicate so I am noting mine for reference – in showing failure to replicate. 
Steps taken to replicate:

1.	Steps: Click on File Choose New next Text Document (Steps performed in version 4.0 and 4.1.1, Windows 7 O/S
2.	Choose the Insert tab and choose Header (default) is the only option available
3.	Choose the Insert Tab and choose Footer (default) is the only option available
4.	Copied a Image into header box
5.	Copied a Image into footer box
6.	Save As .ODT file ext and (Also allowed system to automatically save document)
7.	Reopen saved document
8.	My documents is displayed with saved Images in header and footer without any Read Errors.
9.	Waited an 1 hour (timing) No reverse results in auto saving
10.	I used Internet Explorer and Chorome following above steps to try to replicate

I reviewed the linked bugs (Issue 114371 -migration of incomplete OOo 2 user data fails that note similar issue in different Products and Components of OO. I still couldn't reproduce the bug in the latest veriosn in 4.1.1 due not enough detailed steps and specific configuration and file format.
